1031 Taxpayers should provide the following information:

1. How is the property titled? 
Individual, Husband/Wife, Trust, L.L.C., Corporation or some other way?

2. Address. 
For an individual or married couple, provide the personal residence mailing address. For an entity, provide the registered agent/trustee address.

3. Identification.
For any individual, Social Security Number and Date of Birth is required. For any entity, its Taxpayer Identification Number (FEIN) is required.

4. Closing Company Contact.
Provide the name of the Title Company (or Attorney's office) where the property is closing. Also, provide a contact person and phone number. If a realtor is involved, provide contact information if available.
